Conference Objectives
Upon completion of this conference, participants will be able to:
Clinical sessions
- Understand the inter-relatedness of diabetes and other chronic conditions (e.g.: CVD, stroke, sleep apnea)
- Apply culturally appropriate interventions for patients with chronic conditions
Policy and administration sessions
- Describe new – and recycled – developments in health care reform
- Identify specific opportunities and requirements for Federally Qualified Health Centers

AMA PRA Category 1 CreditTM This activity has been planned and implemented in accordance with the Essential Areas and Policies of the Minnesota Medical Association (MMA) through the joint sponsorship of Stratis Health and Many Faces of Community Health Conference partners. Stratis Health is accredited by the MMA to provide continuing medical education for physicians. Stratis Health designates this educational activity for a maximum of 9.5 hours of AMA PRA Category 1 CreditTM. Physicians should claim credit commensurate with the extent of their participation in the activity.
