Accreditation

This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Minnesota Medical Association (MMA) through the joint providership of Stratis Health and the Minnesota Association of Community Health Centers and Minnesota Department of Health – Health Promotion and Chronic Disease Division. Stratis Health is accredited by the MMA to provide continuing medical education for physicians.

Category 1 Continuing Medical Education (CME) credits will be available. A general certificate of attendance will be provided for continuing education/contact hours. Physicians should only claim credit commensurate with the extent of their participation in the activity.

Attendees are responsible for determining if this program meets the criteria for licensure or recertification for their discipline.

Speaker and Planning Committee Disclosure:

To comply with MMA Standards for Commercial Support, Stratis Health requires presenters to disclose the existence of any significant financial interest or other relationships with companies whose products or services are related to the subject matter of the presentation. Each presenter will submit a signed disclosure form prior to the conference. No presenter or planning committee member involved with Many Faces will have a financial relationship that could be perceived as a real or apparent conflict of interest in the context of their presentation; nor will any speaker discuss research or unlabeled uses of commercial products. 

Physician Continuing Medical Education (CME) AMA PRA Category 1 Credit™

This activity has been planned and implemented in accordance with the Essential Areas and Policies of the Minnesota Medical Association (MMA) through the joint sponsorship of Stratis Health and the Many Faces of Community Health partners. Stratis Health is accredited by the MMA to provide continuing medical education for physicians. 

Stratis Health designates this activity for 5.75 AMA PRA Category 1 Credits. Physicians should only claim credit commensurate with the extent of their participation in the activity. 

CME certificates will be emailed after completion of these steps:

  1. An online evaluation survey will be emailed to you after the conference.
  2. Complete the survey, indicate only those keynotes/sessions you attended. The deadline for completion is November 5, 2020.  
  3. Electronically sign and submit the survey.
  4. A CME certificate will be emailed to you from Stratis Health.
Continuing Education Credits and Contact Hours for Other Health Professionals

The Many Faces Conference may meet continuing education requirements for your focus. Attendees will receive a certificate of attendance following completion of an online evaluation survey. It is the responsibility of the individual to determine if the course fulfills that requirement. The deadline for the completion of the post-conference evaluation is November 5, 2020. Licensed professionals may claim up to 5.75 instructional hours. Those with a 50 minute educational credit hour may claim up to 6.9 credits.
