Accreditation

This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of Stratis Health and the Many Faces of Community Health Conference partners. Stratis Health is accredited by the Minnesota Medical Association to provide continuing medical education for physicians.

Stratis Health designates the 2018 Many Faces Conference for 10 hours (6 hours on Thursday and 4 hours on Friday) of AMA PRA Category 1 Credit™. Physicians should only claim credit commensurate with the extent of their participation in the activity.

Attendees are responsible for determining if this program meets the criteria for licensure or recertification for their discipline.

 

Speaker and Planning Committee Disclosure:

It is the policy of Stratis Health, in accordance with the Minnesota Medical Association and the Accreditation Council for Continuing Medical Education (ACCME), to ensure balance, independence, objectivity, and scientific rigor in all CME activities. Any person engaged in content development planning or presentation must complete and sign a conflict of interest form.

CME certificates will be emailed after completion of these steps:

  1. At the end of each conference day, sign the attendance form at the registration desk and indicate the hours attended. Provide a current email address, clearly printed.
  2. Complete the online evaluation survey that will be emailed to you after the conference. The deadline for completion is November 9, 2018.
  3. Look for a CME certificate emailed to you from Stratis Health within two weeks following the conference.

 


Continuing Education Credits and Contact Hours for Other Health Professionals

Attendees will receive a certificate of attendance following completion of an online evaluation survey. The deadline for completion is November 9, 2018.

Licensed professionals may claim up to 10 credit hours (6 hours on Thursday and 4 hours on Friday).

Those with a 50 minute educational credit hour may claim up to 12 credit hours (7.2 hours on Thursday and 4.8 hours on Friday).
